Issuance of a medical certificate upon request (for existing patients)

We issue a u27 certificate of the patient's health status for presentation at school, before starting work (if tests, chest x-rays and other examinations have been performed beforehand, if necessary), for day and overnight camps or in other similar situations. Before issuing the certificate, a general examination of the patient is performed, vital signs are determined, a pediculosis test is performed (if necessary), a vaccination passport is checked (if necessary). We can also issue a certificate of Covid-19 vaccination (also in situations where the vaccination was received abroad, if this can be proven with relevant certificates) or any other vaccination.
